With the support of more than 30,000 team members, we promote a forward-thinking philosophy that's built on a foundation of trust, dignity, respect, responsibility, and clinical excellence. **Positions details:** + Location St. Joseph Hospital + **Status:** Full-time, salary + **Shift:** 8am-4:30pm + Occasional weekends + **On Call:** no + Always in person The Clinical Nurse Navigator serves as the coordinator for the care of our Ventricular device program, heart failure program. + Responsible to provide support, education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience. + Serves as the coordinator for the care of patients in their specific specialties. + Responsible for facilitating integrated care through communication with strategic members of the interdisciplinary team, collaborating to provide staff education, and works closely with patients, families and members of the health care team to optimize patient satisfaction and achieve optimal outcomes. + Coordinate efforts between physicians, healthcare administration, family, community resources and staff while tracking the progress of the patient throughout their continuum of care. + Promotes evidence-based clinical care that exemplifies the pursuit of clinical excellence. + Assist the congenital/pediatric cardiac anesthesia team with their workflow and make sure that all congenital kids who need any service at SJCH get on their calendar appropriately + Serves as a central access person to the program from the community + This role would support the Ventricular Assist/Heart failure program at St. Joseph Heart Institute **Requirements:** + RN (Registered Nurse) License + Required Bachelor Degree in Nursing + 2 years of acute care experience. 5 years of nursing is preferred + Required Certification in Related Specialty must be obtained within the first 6 months if not current. + A background with VAD experience/heart failure or CVICU education with a background in Cardiac is preferred. _Equal Opportunity Employer Veterans/Disabled_ **Position** Clinical Nurse Navigator- Ventricular Assist/Heart Failure Program **Location** Tampa:St Josephs | Nursing | Full Time **Req ID** 123080
